Under the Capos are their Soldiers who lead the clandestine cells through dead drops and reputation. A Capo typically is the shadowy figure manipulating events within the legitimate organization and in some adventures the Capo is the BBEG. All of these positions are up to you and will determine the shape and scope of control of your crime syndicate. One Capo would be in charge of Importing/Exporting, another in charge of law enforcement infiltration and control, another in charge of criminal enterprises, another in charge of legitimate business operations. He would be the Boss, with an appropriate underboss (monstrous creature, intelligent, loyal, easy to remove if necessary but strong enough to keep the others in line, possibly a Mind Flayer), the Underboss would have their direct subordinates or Caporegimes that would split out the operation into spheres of influence. Your Beholder would, in the core of his domain, have the mafia set up in his followers. If you like you can superimpose a Mafia hierarchy to the top of a cell structure and in this way make a syndicate that encompasses many disparate operations both illegal and legitimate.
